Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, I understand that there is risk of serious damage or injury to another dog, it's owners/handler or other persons or animals, my guest(s), my property, my dog(s) and others in the park. I hereby agree to assume these risks for my dog(s), my guest(s), my property and myself.

I confirm that my dog is spayed/neutered (if over 1 year of age), and is current on Rabies and Distemper Parvo vaccines. I have provided proof of these services through my veterinarian (if not a client of GrandView Animal Care Center).
